"Using Electronic Health Record (EHR) Data and Other Methods to Improve
Care: Lessons Learned".
Dr. Tashjian will discuss how a team-based approach to care can benefit both
practice and patients alike. Working together, clinicians can make the most
of technology to best support strategies for improving health outcomes. The
difference between care coordination and chronic disease management as they
relate to blood pressure management also will be discussed. Q&A follows
the presentation.
