Bellin Health uses a team care model that includes physicians,
nurse practitioners, physician assistants, licensed practical nurses and
medical assistants to not only advance the health and wellness of their
patients, but also of their staff. The patient and family is set at the
center of the care team, which results in improved professional
satisfaction and engagement among the staff as well as improved outcomes
for patients and better communication and teamwork. Learn about how to
incorporate this model into your practice
